Serapha Bio Announces Positive Interim Safety and Efficacy Data from the SERP-01 Investigator-Initiated Trial Presented at the ERS Alpha-1 Antitrypsin Conference 2026

  • Single administration of SERP-01 was associated with increases in total serum AAT above the protective threshold with durable correction out to 10 months
  • Elastase inhibition assay results showed serum approached activity of healthy donor serum
  • No serious adverse events and no treatment-related adverse events of grade 3 or higher observed as of the data cutoff
  • Serapha expects to initiate a U.S. Phase 2/3 trial of SERP-01 in late 2026 or early 2027

NEW YORK, Oct. 26, 2026 (GLOBE NEWSWIRE) — Serapha Bio, Inc. (Serapha), a clinical-stage genetic medicines company committed to transforming the treatment of alpha-1 antitrypsin deficiency (AATD), today announced early clinical data for YOLT-202 (SERP-01), an investigational one-time in vivo base editing therapy, from the ongoing investigator-initiated trial (IIT) evaluating its safety and efficacy in four participants with the PiZZ genotype. Simon Krooss, M.D., Ph.D. of Hannover Medical School presented the data at the European Respiratory Society Alpha-1 Antitrypsin Conference 2026, held on October 23-24 in Barcelona, Spain. The IIT is being conducted through a collaboration between Renji Hospital in Shanghai, China and Hannover Medical School in Germany.

The presentation, titled “Clinical Proof-of-Concept for In Vivo Base Editing in PI*ZZ Alpha-1 Antitrypsin Deficiency: Results from the YOLT-202 (SERP-01) Early P1 Trial,” described data that support the potential of SERP-01 as an efficient and safe one-time gene editing therapy for AATD.

“While early, these positive data look very promising. The precision of the editing profile and the observed restoration of circulating AAT levels reinforce our confidence in SERP-01’s potential to be a best-in-class therapy,” said Kenneth T. Mills, Chief Executive Officer of Serapha. “We believe these early findings provide an encouraging foundation as we work to advance SERP-01 through clinical development. We are preparing to initiate a Phase 2/3 clinical trial of SERP-01 in the U.S. in late 2026 or early 2027, driven by our commitment to the many people living with AATD who continue to wait for a one-time therapy that addresses the underlying cause of this disease.”

As of October 15, 2026, the data show that in the ongoing IIT, a single administration of SERP-01 was associated with rapid, dose-dependent increases in total serum AAT above the 11 µM protective threshold in all four participants (n=1, 35 mg; n=3, 45 mg), and a shift in circulating AAT from predominantly Z-AAT to predominantly M-AAT (98.7% at 35 mg and 93.4% at 45 mg by week 3, measurements approaching the ceiling of the liquid chromatography/mass spectrometry assay). Total AAT measured by turbidimetry in the 45 mg cohort was at or near the 20 µM lower limit of normal through up to 10 months of follow-up. The function of this total AAT was assessed by an elastase inhibition assay performed at one and two months after infusion, in which serum from three of four participants approached the activity of pooled healthy-donor (PiMM) serum, and all four exceeded the activity of PiZZ control serum. In a liver biopsy obtained at 10 weeks following treatment from the participant dosed at 35 mg, DNA sequencing showed correction at the target site (54%) with no bystander editing detected at neighboring bases. To Serapha’s knowledge, this is the first reported assessment of hepatic gene correction using a lipid nanoparticle-delivered base editor in a patient with PiZZ AATD. No serious adverse events or treatment-related adverse events of grade 3 or higher were observed as of the data cutoff date. Longer follow-up and additional participants are needed to assess safety and efficacy of SERP-01.

“In these first patients, a single administration produced rapid and sustained increases in circulating AAT, and almost all the measured protein was the functional M-AAT form that resulted from precise, high-efficiency editing, as stated via liver biopsy,” said Dr. Dr. Krooss. “People living with AATD face progressive lung and/or liver disease, and there is a clear need for new treatment options. These results are an encouraging step, and we are committed to advancing this trial for the AATD community.”

About SERP-01
SERP-01 is an investigational in vivo base editing therapy designed to correct the most severe disease-causing SERPINA1 E342K (PiZZ) point mutation at its source, restoring production of functional M-AAT while reducing the toxic Z isoform of alpha-1 antitrypsin (Z-AAT), with the potential to address both the liver and lung manifestations of AATD with a single intravenous administration. SERP-01 is currently being investigated in the ongoing IIT to evaluate safety, pharmacokinetics and efficacy, including multiple secondary outcome measures. The U.S. Food and Drug Administration (FDA) has cleared an Investigational New Drug Application (IND) for a Phase 2/3 clinical trial of SERP-01 in PiZZ AATD patients. Serapha intends to initiate this trial in late 2026 or early 2027. The FDA has also granted SERP-01 orphan drug designation and Regenerative Medicine Advanced Therapy (RMAT) designation for the treatment of PiZZ AATD. Serapha holds exclusive rights to develop, manufacture and commercialize SERP-01 outside Greater China under a license from YolTech Therapeutics.

About Serapha Bio
Serapha Bio is a clinical-stage genetic medicines company developing a proprietary base editing therapy, SERP-01, for patients with AATD, the leading genetic cause of serious, progressive lung and liver disease. SERP-01 is in clinical development for the treatment of AATD patients with the PiZZ genotype, the most severe genetic mutation responsible for >90% of AATD cases. SERP-01 consists of a lipid nanoparticle formulation containing messenger RNA encoding Serapha’s proprietary adenine base editor together with a guide RNA. The therapy is designed to enable precise correction of the disease-causing mutation in PiZZ AATD. Correcting PiZZ, the most common gene variant associated with severe AATD, could address a large population in the U.S. and Europe.
